- Confirm that you completely fill all fields marked with * Pastikan anda mengisi setiap maklumat yang bertanda star dengan lengkap

- Fill completely Isi sampai habis
- Click "confirm & send" Klik "sah & hantar"
- Print the application slip Cetak slip permohonan
